I doubt that’s possible with the massive shoulder on the back side. It was in there finger tight.

CVA said overpressure… “they only recommend 2F powder” not 3F. Easy cop out IMO.

I think it was a bad plug from the get-go. Order another.
 
It was fine before I shot it last. Not belled at all and went in and out easily by hand like they’re supposed to.

I suppose it could have been thin in that area from the factory MrTom. Didn’t think about that, but it didn’t look markedly different from the others I’ve had in the past.
 
The shoulder on the finger nut would stop the plug from going in so far as to cause belling like that. I think the plug wasn't finished at the barrel end to the proper length. Send that one back and request a new one.

You don't mention what load you'd been shooting either. Knowing the powder, the charge size and bullet weight can also help here. A mega charge with a medium weight bullet might cause this too. What are the load details?
