Not my intention to point fingers but are you sure that you remembered to check the witness mark? The worst thing at the range is distractions and interruptions.
If you have to push real hard to slide a slightly oversize bullet down the tube, is the bullet more likely to 'stick' to the ramrod and come a little ways back up when removing the ramrod?The bullet most likely migrated up off of the powder charge. Probably stuck to the ramrod tip. Unfortunate, but it can happen if you don't check for it. Powerbelts and other bullets like them are easy to load but that easy loading comes at a price, if they go down easy, they can move up easy too.
